This is the fifth months of me, passing in Japan. This is the month of February, mother language month. In 21 February, 1952 many people of Bangladesh died for the recognition of Bengali language as an official language. In 1999, UNESCO declared 21 February as international mother language day in tribute to the martyrs of the language movement and the ethno-linguistic rights of people around the world. I respect and honour the Japanese people as they are very comfortable to speak with their mother language.

In this month I am very lucky to see, Funnel chest operation and many VATS lung and mediastinal surgery. This is the 1st time, I seen the chest wall deformity surgery, where Pectus excavatum deformity was corrected by Dr. Kasagi, here he applied the Nuss procedure (The minimally invasive repair of pectus excavatum) In Nuss procedure, the correction of a chest wall deformity through a prolong action of a pushing force exerted on the anterior chest wall by a special metal bar is applied.

In 14th February 2018 I was in Kawasaki, in Covidian Lab where surgery is done in animals. I had done left upper lobectomy successfully with the help of Dr. Aoshima and Dr.Yoshiya. I am thanking professor Kanzaki and Covidian Company for their nice arrangement. I am also thanking Dr.Aoshima and Dr. Yoshiya for their hearty co-operation.

I am participating every day morning conference, where preoperative and postoperative management of patients are discussed, evaluate the patients for further managements. I also participate in Journal presentations and weekly professors round.

In this month I visited the imperial palace of Tokyo, is located on the site of the old Edo Castle, the residence of the shogun the location of the Tokugawa Shogunate. When it was completed approximately 400 years ago, Edo Castle was the biggest wooden building in Japan at a height of 60 meters. When Japan entered the Meiji period in1868, the old Edo Castle became the Tokyo Imperial palace. It is the residence of the emperor and empress as well as the main palace hall where ceremonies are held. I also visited the Tokyo tower that came into operation in 1958.Tokyo tower is higher than Eiffel tower of Paris. It looks beautiful at night due to wonderful light projection.

I am still enjoying the Japanese cuisine sushi, tempura and bento at the weekend. This month was almost dry, but biting cold and windy weather made my life miserable though I am enjoying my life with the great Japanese people.

This is the month of January, 2018, fourth months passing in Japan. After spending the vacation of Marry Christmas and happy New Year, 2018, I started working in my surgery 1 department. I am participating morning clinical session, journal presentation and weakly round where preoperative workout, postoperative care and postoperative complications of patients and their management are discussed.

I am seeing many VATS lung surgery as well as mediastinal mass surgery. I was lucky to see one primary lung cancer infiltrating the pericardium, where involving part of pericardium was removed along with lung cancer. The part of pericardium was replaced by artificial pericardial membrane. I have not seen this major type of surgery previously. I had also an opportunity to see the two anterior mediastinal tumours both were Thymomas were removed by VATS in lateral position. I had also an opportunity to see VATS decortication, that is removal of pleural peel from the visceral pleura, evacuation of pus, debris deposited in the pleural spaces. Professor Kanzaki, Dr Murasugi, Dr Oyama, Dr Matsumoto, Dr Isaka, Dr Maeda, Dr Aoshima, Dr Katagiri, Dr Yoshiya, Dr Mitsuboshi, Dr Soga, and Dr Takeshma are helping me in each and every moments. I am very grateful and pleased to them.

I have had a great experience of seeing snowfall in this month, and it was January 22, 2018. A heavy snowfall hits the Tokyo, started from the morning till middle of the night. The city turn into a dazzling white coverings. Roads, roofs, fields, trees every things were buried by soft white foamy snow. Children were playing with the snow, making ball, statue and many other sculptures. I was very happy to see the snow fall and really enjoyed it very much.

The weather of this month was almost dry except snowfall and some drizzling of rain. I visited the Sky Tree of Tokyo and Aquariums of sky tree town at the last weekend of this month. The Sky Tree of Tokyo is the highest tower of Japan and third highest tower of the world. It is very beautiful place in Tokyo and more beautiful at night due to different colour of light projection. In Aquariums different types of rare beautiful fish, turtles and plants were preserved. The different type of jelly fish with their reproduction was really charming and astonished me. I had introduced here with different type of cat fish, box fish and different coloured eel fish which was very new to me.

I am enjoying the Japan by introducing with its very rich and ancient cultural activity, sports and different taste of foods. I am also enjoying the Japan by mixing with great Japanese decent hard working people. I have learned many things from them.

This is the third months of passing in the Japan. When I came here in Japan, I was like, I had a great problem as a new comer, but as time goes I am now able to cope up the new environment, new colleagues, new people and new cuisine. In November I had a chance to see a lot of VATS (Video Assistated Thoracoscopic Surgery). I was lucky to see few open thoracic surgery like total excision of sternum and closer by proline mesh, open thymectomy by median sternotomy.

I had also an opportunity to see a patient of haemoptysis (coughing of blood) with its subsequent management. It was an exceptional case. Her haemoptysis was due to collateral blood supply from nonbronchial arteries via transpleural vessels from infradiaphramatic branches of left gastric artery. Interventional Radiologist after angiogray did Embolization of collateral branches of left gastric artery by using proximal blocking agent, detachable balloons and stent grafts. It was a new experience for me; I have never seen this type of patient before. But this patient continues to bleed after embolization that is her haemoptysis did not stopped. Later on she underwent VATS left lower lobectomy, her postoperative period was uneventful. The patient went home with smilling face and this is the beauty of thoracic surgery. I had a chance to see Robotic surgery, thymectomy was done .This is also a new experience for me.

In 7th December I went to Kyoto to join to Japan endosurgical conference. Lot of papers on thoracic surgery was presented there. Our Professor Kanzaki presents papers on navigational VATS segmentectomy, it was an excellent presentation. During stay in Kyoto I visited many historical places and Temples.Dr.Matsumoto helped a lot during this visit. Dr. Murasugi invited me a dinner party during stay in Kyoto. I am greateful to him for introducing me to many Japanese cuisine. In 14th December was TWMU grand dinner party, I enjoyed it very much.

I am learning some Japanese greetings, having some Japanese foods like Sushi, Sashimi, Bentos, Tempura, Nave, Oden soup, Soba and different types of sea foods and vegetables. I also like to eat different types of fishes like Tuna, Salmon, Red seabream and so so.
In the meantime I visited Nara, Kamakura, Yokohama city, Enoshima. I also visited Ueno zoo to see the Panda and Polar bear, which I never seen before. I visited different parks of Tokyo to see natural beauty of autumn that is colour changes of leaves of trees. It was awesome and mind blowing, I enjoyed it very much

In 16th December 2017 I went to my country to spent vacation of Christmas day and happy New Year with my family. My wife, daughter and son became very happy to see me and I enjoyed my vacation. I came back to Japan on 8th January, working now with full attention.

Introduction

I am Dr Mohammad Osman Goni working as a fellowship trainee in Tokyo Women’s Medical University, Tokyo, Japan. I am working here from 1st October 2017.One day probably first week June 2017, I was informed from my head of the department associate prof.Kamrul Alam if I could join a training program in Japan, in Thoracic Surgery in TWMU. It was my dream to attend a training program in Japan. Instantly I agreed with my teacher. Then 1st week of July I had a meeting with my principal, vice principal and Dr.Alimuzzaman of JBMA by which I was selected to come for training in Japan. In 27th September 2017, I leaved Bangladesh. In 28th September 2017, Maiko Kunimi received me from Narita airport, that surprised me. I feel it’s a great opportunity and privilege for me to join as a fellowship trainee in TWMU.
I am grateful to JBMA Japan, International affairs of TWMU, Professor Sakurai sensei, Dr. Alemuzzaman and special gratefulness to Maiko Kunimi and Etsuko Yusa for doing everything for me.
Many many thanks to Dr. Horidas Saha and Dr. Zakaria Jubery for their guide to me in Bagladesh.
Finally thanks to Professor Kanzaki and doctors of his department surgery 1, received me with kind enough and arranged a welcome party for me that astonished me and that day I was introduced with Japanese cuisine and foods

Clinical activities in my department
In department surgery 1, there are routine operation on Monday, Tuesday and Friday in a week and clinical session in the morning except Sunday. In first day Professor Kanzaki introduced me with doctors of his department and asked my intention to work. I told my intention to work with VATS, then he said 95% of operation is being done here by VATS.I become happy and honoured as he gave permission to work here. Dr.Matsumoto, Dr.Maeda and Dr.Yoshiya arranged everything for me
I could not believe that I am going to see all VATS procedure like VATS lobectomy, VATS partial resection, VATS Mediastinal lymph node dissection, VATS thymectomy with 3G camera.
Last month I also observed two open operative procedure that are total excision of sternum and thymic carcinoma operation. First time I was experienced with VATS diaphragmatic traffic and VATS muscle flap for empyema thorasis with bronchopleural fistula. Here operation theatre environment is excellent and preoperative workout for patient is remarkable as well as outstanding. I became astonished how they reached very inside of the diseases by studying CXR, CT scan, MRI, PET scan.
Sacrifice, commitments and devotions of Professors, doctors, nurses and others to the patients is inexpressible. So many operation I observed here that is tremendously helpful for me.

Academic activities:
Every Wednesday day and Thursday is our morning clinical session and journal presentations also every Saturday morning professors round. I am taking part in morning session and journal presentations and discussions regarding patients managements.

Social and Communication:
On arrival in Japan I am experienced heavy rain almost every day. I already visited mount Fuji, Shinjuku national garden, Dr. Matsumoto parents house Gunma and Yokohama. I am mixing with Japanese people, the people here are very hard working, gentle, decent, cooperative and kind enough. I am also trying to learn some Japanese greeting
