When this report is posted may be I am far away from here and have reached my sweet home. It was almost one year I stayed here in Japan and it was full of memories for me. When I came here at Japan I was like, I have fallen in a sea. But as the time goes I was able to cope up with the new environment, the new culture, new people and new cuisine. Definitely now I am dare to say, I am going to miss Japan. I love Japan, I love the people of Japan. I just love this country. I am really grateful to my sir Prof. Dr. Md. Abul Kalam sir, Dr. Samanta Lal sen sir, Prof. Dr. Md. Sazzad Khondoker sir, Prof. Dr. Rayhana Awwal madam for selecting me and giving me the opportunity to attend this training programme. My heartiest thanks goes to my mentor Prof. Hiroyuki Sakurai sir for accepting me as a trainee and he has taught me a lot of things. I will definitely miss him a lot. My special thanks to Prof. Emeritus Motohiro Nozaki sir, he is a guy I would always miss. My gratitude goes to the department specially Dr. Yamaki sensei, Dr. Itoh sensei, Dr. Matsumine sensei, Dr. Fujiwara Sensei, Dr. Kikuchi sensei, Dr. Tanabe sensei, Dr.Yamamoto sensei, Dr. Honda sensei, Dr. Takeuchi sensei. I would like to thank Dr. Masa Hasegawa, Dr. Yuki Hasegawa, Dr. Osada, Dr. Kento Homma, Dr.Wataru Kammei, Dr. Mariko Mogami, Dr. Tatetsu, Dr. Sasaki, Dr.Iwata, Dr. Mizobuchi, Dr. Hagiwara, and Dr. Fujii.

The guys without whom I could not do anything, the guys who were here whenever I need anything, whenever I need any help Dear Maiko Kunimi san and Yumiko Shionoiri san. I am really grateful to them. I also would like to thank Fumiko Daikoku san and Motoda San. I would like to thank all the doctors and staff of other department the OT nurses and other staffs, without their support I could not complete my training here.

In last one year I had a chance to participate a lots of plastic surgical cases. Some of those were new to me. I have seen a lots of Breast reconstruction cases here and it’s because Prof. Hiroyuki Sakurai sir is very good to do that. A lots of Free flap cases ,Expander implant cases, different types of flaps, burn wound management, post burn case reconstruction, congenital cases like cleft lip and palate, retrosternal bar for pectus excavatum, varicose veins, maxillofacial fracture cases, replantation,lymphovascular anastomosis for lymphedema treatment, nasal bone fracture,hernioplasty, eyelid surgery,ear reconstruction,vascular lesion cases,different types of scar revision, excision of lumps and bumps,application of laser, perforator flaps, pressure sore management and so so. It was nice to learn some different techniques and hope that I can use my experience here while going back home.

I had chance to attend some conferences while staying here at Japan. Also had a chance to present my poster at plastic surgical conference at Osaka. I had chance to visit some places of Japan. The prefectures I visited like Hiroshima, Osaka, Kyoto, Kobe, Niigata, Saitama, Shizuoka, Tochigi, Chiba, Yamanashi, Kanagawa, Ibaraki. I could visit the different places of Tokyo and I really enjoyed my staying here at Japan.

Have learnt some different culture, different cuisine. Fooding was very difficult for me when I came here but day by day I could manage myself. Specially as a muslim I never tried alcohol, pork and meat(if it is not Halal).I also have milk and milk product allergy, and it made my days miserable as having food that contains milk or snacks or bakery, sweets was not possible for me. So in that sense I could hardly eat food outside and I always had to cook for myself.

I tried some Japanese food here like Sushi, sashimi(raw fish),different types of bento, Tempura, Nabe cuisine, Oden soup, Ramen, soba, Okonomiyaki, different types of sea foods, vegetables. I liked to have fruits here and also fish like Tuna, salmon, red seabream (Thai) and so so.

Could enjoy some traditional Japanese culture too.

The people here are very hardworking, honest, sincere, polite, and gentle. I like the way they treat other people and I definitely like Japan. I will definitely miss Japan and will miss PRS TWMU.

My special thanks goes to my Family , my wife , my daughter , my parents and others . Without their support and sacrifice I could not complete my training here at Japan.

I was excited about Dr. Sen sir’s visit to Japan. The golden week added some cherish able memories for me in Japan. The very pleasant weather of May along with some traveling ,joining conference all were what I had in this month. Meeting Hirayama sensei again and talking to him would be my lifetime sweet memory. Knowing more about him gives me inspiration to do something more for my country.

Hospital Activities:

Not too many operating days for me in this month. But there were free flap cases. All free flap cases was to reconstruct breast and not to mention those were DIEP free flaps. And I am getting inspired day by day to see the decision making and techniques of Sakurai sir, how good he is to reconstruct breast! Its amazing. One of the breast reconstruction was using free gracilis perforator flap. Though initial planning was to do TUG(Transverse Upper Gracilis Flap).But as there were good perforators along the gracilis muscle so Sakurai sir changed the plan and did gracilis perforator free flap. Revision case like breast after DIEP flap, to change the volume with liposuction and reshaping to have a better symmetry. Eyelid reconstruction using TFL sling and blepharoplasty cases we had in this month. Expander implant breast reconstruction and removal of ruptured implant cases was in this month. Tough ruptured implant cases are not that very common. Amputation case like midarm amputation and below knee amputation case was in the schedule. Doing the amputation always hurts me, but when it is necessary to save life and limbs its what we had to do. Removal of retrosternal bar for pectus excavatum and some revision of scar cases was also. Secondary lip revision and definitive rhinoplasty cases was very nice to see as in our country we are still lagging behind to do rhinoplasty cases.At the last week of month I had a chance to visit TWMU Yachiyo Medical center at Chiba. Prof. Masaki Takeuchi sensei was very kind and cordial to give me the chance to visit the PRS department. The hospital is very nice and infrastructure is much better than its mother hospital .I had a chance to see papilloma removal case and distally based sural flap along with STSG .Have seen new technique for sural harvesting there and hope to do so while returning back to my country. Blepharoplasty technique of Takeuchi sensei was suprerb and I really like the way he did the case. In the middle of the month Dr. Sen sir(President of Society Of Plastic surgeons of Bangladesh and Also the National Co ordinator of Burn unit Projects Bangladesh) delivered a lecture in an auspicious gathering here at TWMU. He showed the history of plastic surgery of our contry and also the present and future of the subject in our country. Also joined the Burn meeting at the last week at Keio plaza hotel. Met Prof. Pernelel Enkhbaater sir there. It was very nice to meet him again there.

Extra academic activities:

The month started with the golden week and at first I did not have a plan to go somewhere. But at the middle of the week we planned and visited some nice places in Japan using special JR ticket for foreigners. Went to Ibaraki, Hitachi sea side park and was overwhelmed to see the beauty of Nemophila bloom. The flower field, the blue sky and the sea all were in a single canvas and definitely it was one of the best beauty for me in Japan. We also went to Shimoda of Shizuoka prefecture and visited ropeway, beach and aquarium. The sea ,the mountain all so beautiful! At the very last day we went to Nikko area’s cultural heritage sites and waterfall . The golden week really added me some sweet memories. Had a chance to go to airport to receive Dr. Sen sensei and I was really happy to see him here. Got the opportunity to visit Mt. Fuji and Hakone with him. And of course the sweet Maiko San was very kind to be with us. Eating Kuro Tamago(Black Egg) with Maiko san at Owakudani was really fun for me tough the sun did not show up there. So it was windy and foggy. But still I enjoyed being with them. Had a chance to go with Sen sensei to meet Prof.Hirayama Sensei and his family. It was a rare chance for me and talking and knowing more about him gave me a lot inspiration to do something better for the distress people of my motherland. Me and Dr. Afroza planned to cook some Bangladeshi food for the department and we had a curry party on 22 May. I hope our teachers and doctors and friends here could taste some Bangladeshi food at the party.

When I am writing this report I actually is counting for my departure day and hope I will write my last Report before going back to my home,sweet home.

The best month so far for me in Japan is April. It was full of fun, work, cherry blossom viewing, conference, dinner and so so.The beauty of nature and the beauty of Japan are best viewed this month. The weather was so pleasant .The Bengali New Year is also observed in this month and we enjoyed a lot Boishakhi mela at Ikebukuro. Dr. Afroza came at the very first day of this month. And it was very nice for me.

Hospital Activities:

Having seeing some free flap cases was as usual for me in this month. Free iliac crest flap for treating ameloblastoma of mandible was very nice and I could see free iliac crest bone flap for the first time. Sakurai sensei is very good with his planning. And I always love to see the way he plans to do any reconstruction. LD free flap cases for leg ulcer and sarcoma was in this month. Itoh sensei did the cases very nicely. CA breast case with both DIEP and SIEA case was done but SIEA flap was lost unfortunately and the following week free LD flap was done to recover the loss. Expanded free DP flap case was done for facial scar treatment. We had some good cases of Lymph vascular anastomosis for lymphedema treatment. Sacral sore was treated by rotation flap. Abdominal Incisional hernia case was operated as herniorraphy .ORIF cases for orbital floor and maxilla fracture, retrosternal bar placement for pectus excavatum . Some STSG and FTSG cases. Amputation of limb for PVD and expander implant exchange for CA breast cases was as usual as other months. Could see some outdoor cases like eyelid surgery, lip lesion excision and some local cases. The best part of this month for me was attending the 60th annual meeting of Japan Society of Plastic and Reconstructive Surgery at Osaka. I was privileged to present a poster there. The title was ‘Midline crossover Venous Connections in Transferred Lower Abdominal Flaps for Breast Reconstruction’. I am really grateful to my mentor Prof. Hiroyuki Sakurai sir for giving me the opportunity to present and for instructing me to prepare the poster. Could see some innovative ideas and methods in the conference and enjoyed the invited lectures as those were all in English.

Extra academic activities:

Cherry blossom viewing was the most fun so far for me. We had a Sakura viewing program from the department at Ueno Park followed by dinner. Having dinner sitting under the cherry trees was so good. I really enjoyed. Me, Dr. Saha and Dr.Afroza went to some places to see Sakura. Shinjuku Gyeon national garden, Sumida park, Sumidagawa river, Ueno park and in Osaka. And I would say I enjoyed the cherry blossom viewing a lot. Bengali New Year celebration at Ikebukuro was great. As for the first time I could see so many people of Bangladesh altogether. There were food stalls, cultural program and so on. We enjoyed the whole day there. During the conference time I could visit Osaka, Kyoto and Kobe (Though very short visit) .And I really enjoyed .A lot of dinner invitation was in this month. Sakurai sensei invited us to a Bangladeshi restaurant for dinner on the day Dr.Afroza came. And it was all Bangla cuisine like Alu Vorta (smashed potato with herbs), Begun Vorta (smashed Eggplant), Dal, Chicken curry, mutton curry, and rice and so on. The food was Oishi. In Osaka we joined Alumni association dinner in a Japanese restaurant .We had another dinner invitation from Aleemuzzaman sir at his residence. Dr. Noriko Sensei (Better half of Aleem Sir) prepared some Bangladeshi food for us. I was really surprised to see all Bangladeshi food there. The food was 10/10 and thanks a lot them for the dinner. We also joined another Alumni program at Toshimaen. It was a hot spa and onsen bath and relaxation followed by traditional Japanese dinner. Very last week we had another dinner invitation from JBMA in a tempura restaurant. Tempura is one of the foods I like here in Japan and I really enjoyed the dinner. I want to thank Prof. Nozaki sensei, Prof. Sakurai sensei, Dr. Yamaki sensei, Dr. Itoh sensei, Dr. Matsumine Sensei and Dr. Fujiwara sensei for the dinner. So it was the month of fun, Full of Sakura viewing, invitations and so so.(The most number of invitation and outings was in this month so far for me!)

The most awaited thing for me was cherry blossom in Japan. And as the weather was a bit pleasant for me as well as the cherry buds coming in the trees was so good to explore in March. The ski trip to Mt. Naeba in Niigata with the department was a very memorable tour for me.

Hospital Activities:

I could get some experiences from some good cases in this month. DIEP free flap case was as good as Sakurai sensei has masters the technique. He also did breast reconstruction with SIEA free flap. Lateral thoracic artery based free flap for reconstruction of elbow after scar contracture release was new experience for me. As for the first time I have seen this type of free flap .We had a gracilis flap for coverage of excised valvar carcinoma. For the first time I also could see Bilateral Saggital split osteotomy(BSSO) for prominent mandible case by Honda sensei. Breast reconstruction case with either expander placement or expander implant exchange cases were in this month. Still learning from every operation. Expander placement over leg for scar revision case was in this month. I also could see some maxillary fracture cases. Also some lipoma, dermoid cyst, schwannoma excision cases. Entropion correction with hard palatal mucosal graft was very nice to see. I have seen Etoh sensei performing excision of aneurysm with so good skill. Parotid tumour excision, STSG, FTSG, revision of neck contracture cases were as usual. Have seen some cases in outpatient operation theater. Scar revision, lipoma excision, Z plasty for epicanthal fold, double eyelid surgery and other cases. Had a chance to visit Nippori clinic and could see botox injection, filler and laser treatment. I am grateful to Prof. Sakurai sensei, Dr. Honda sensei, Dr. Kure sensei and Dr. Goto sensei for giving me the opportunity and helping me to understand the procedures.

Extra academic activities:

Skiing trip to Niigata was a very memorable tour for me. As for the first time I could see snow and tried Ski gears! I would like to thank Prof. Nozaki sensei, Prof. Sakurai sensei, Dr. Osada sensei, Dr. Tatetsu sensei and Dr. Sasaki sensei for the trip. I want to give special thanks to Mariko Mogami sensei for the trip. She was very kind to take care of me during the whole trip. She also helped me with the Skiing gears and trained me a bit! Though I am not a good Ski student! The very sad part of this month was farewell of Dr. Mogami, Dr. Sasaki and Dr. Tatetsu. Definitely I will miss them. They helped me a lot in the last few months .I will miss the energetic guy Dr. Tatetsu. He is so lively and always tries to enjoy his day. Hope them for the best. In the very last week Mr. Benjamin a final grade medical student from USA came here. I could exchange some views with him. Also had sightseeing with Dr. Haridash Saha in weekends. Waited to explore cherry blossom in Tokyo which is to full bloom in 1st week of April as per forecast.

The winter continues this February too. It’s really cold month but a lot of sunshine too. And the sunrise time is getting back, so when I start from my place it’s still dark. Had good operative schedules. Had times with Dr. Haridas. And now he is getting used to new environment. Went some places in Tokyo with him in weekend. But the most important fact that I missed this month is the Plastic surgical conference of my country.

Hospital Activities:

This month we had some interesting cases. Free flap cases like LD free flap case for skull bone coverage. And for flap harvesting Sakurai sensei was so expert to spare vessels for Serratus anterior and thoracodorsal nerve. And I enjoyed his techniques. DIEP free flap case for breast reconstruction was in this month. In one of the cases patient wanted augmentation of opposite breast. So Sakurai sensei planned the operation in such a way that he used the same DIEP flap for augmentation of opposite side. The donor flap was divided during harvest and one of the parts with DIE vessels was used to augment the opposite side and was tunneled and anastomosis done with reconstructed side flap pedicle. It was a very good idea to reconstruct. We had pedicle LD flap for shoulder coverage. Some Skin grafting cases like STSG, FTSG. Open reduction and internal fixation of different types of maxillofacial fracture cases. Nipple reconstruction with arrow flap case during DIEP flap was in this month. Placement of tissue expander and use of rotation gluteal skin flap for sacral pressure sore case was there. There was open rhinoplasty case. It was the first time I have seen open rhinoplasty case here. Also secondary rhinoplasty for cleft patient. The use of artificial bone matrix to augment alar base was first time for me. Tanabe sensei is very good with cleft cases and she also used columellar strud graft and tip rhinoplasty. Yuki Hasegawa sensei helped me to understand the procedures. There were some cases in outpatient clinic. Excision of mole, blepharoplasty, scar revision, Z plasty for hypertrophic scar and other cases too.

Extra academic activities:

As Dr. Haridash arrived here last month so this month I had to take him to different places of Tokyo. We went Sumida area. He really enjoyed the river and the breeze. We went to East garden of Imperial palace and Marunocchi area. Also visited the Meiji Jingu Shrine and Yoyogi Park. On 21st February we went to Ikebukuro-nishiguchi park. It was the day of international mother’s language day (আন্তর্জাতিক মাতৃভাষা দিবস). There is a permanent monument of the replica of SHAHID minar of Bangladesh there. A lot of Bangladeshi people were there and showed there solitude by placing flower bouquet there and singing the special song designated for the day, Amar Vaiyer Rokte Rangano Ekushe February, Ami ki vulite pari?( Watashi no otōto no chi no ETV 2 tsuki wa, watashi ga o susume?).
In the later part of the month we enjoyed Tokyo marathon from Tokyo Metropolitan Government area at Tochomae. At the very first week I had a chance to go out with Sakurai sensei, Iwata san and Maiko San to see the conference venue of 2019 Microsurgical conference at Shinjuku. And we had dinner in an Indian restaurant named Ashoka. It was one of the best dinners I had here at Tokyo (as they had all Indian Menu). I really enjoyed the dinner. My heartiest thanks to Prof. Sakurai sensei, Ms Iwata san and Ms. Maiko Kunimi san. Last but not the least; I really missed the plastic surgical conference of Bangladesh.

Happy New Year and Its too cold! And this is what we are not used too! The first week was almost a holiday and not a lot to do. But later on we had some interesting operative schedules. In the middle of month Dr. Haridas came from Bangladesh. And at last I have someone to talk and share!

Hospital Activities:

The second week onwards we had busy operative schedules. Some free flap cases. DIEP free flap cases were in this month and I continued to learn new innovative from Sakurai Sensei and so lot to continue. For the first time I could see deep circumflex iliac artery perforator free flap for breast reconstruction. Also had SCIA free flap for reconstruction of preauricular area and part of contracted auricle. Was so enthusiastic to have Itoh Sensei working on LD flap breast reconstruction and some breast reconstruction with implant. Having some ideas from facial reanimation operation cases by Matsumine sensei and he did right sided facial nerve palsy reanimation with TFL sling. Had chance to see vascular anastomosis in hepatic transplantation. Got some refreshes with nipple reconstruction with CV flap and arrow flap. Placement of retrosternal bar in chest deformity case was so visually appreciable and the result is just to see at the time of operation. Kikuchi sensei is so efficient to his techniques. In case of lower eyelid reconstruction use of palatal mucosal graft was new for me. Cases like ORIF, release of PBSC and FTSG were very nice to see. In outpatient operation room had some scar revision cases, lower eyelid tumor excision case, blepharoplasty case, excisional biopsy cases. I am very grateful to Prof. Sakurai Sensei as I could see his innovations even in scar revision cases. I can learn a lot from his way of thinking, his way of planning cases.

Extra academic activities:

The winter breeze was so strong this month and it was hard to roam around the city. Even in the first week the shops were closed. I am grateful to Prof. Nozaki sensei, Prof. Sakurai Sensei, JBMA and Maiko San for the wonderful and warm winter gift. The down coat is really nice and keeps me warm in the strong winter breeze. But the very nice thing to me to have Dr. Haridash from Bangladesh. I had chance to receive him from airport. Actually it was one of the coldest days of the month when Dr. Haridash arrived and he took that trouble to adapt himself in this cold. The next day of his arrival it was a nice meeting of Alumni Association in Keio Plaza hotel. We could attend the dinner party and it was a rare chance for us to have senior plastic surgeons around Japan who are TWMU alumni. We met Prof. Hirayama sensei, Prof. Nozaki sensei, Prof. Nakazawa sensei, Prof. Takeuchi sensei in that meeting. I had to take care of Dr. Haridas for his first visit to his surgical gastroenterology department and we met Prof. Yamamoto sensei in his office. Next few days we walked and went shops, convenient stores so that Dr. Haridas gets acquainted to local foods and his groceries.

I was really excited for this month as Prof. Sakurai sensei permitted me to visit my family in Bangladesh. My daughter was to start her school admission and I had to shift my residence near to her school. It was quite a pleasant journey for home and my daughter was really happy to have me. While my staying in Bangladesh for my short visit I had a chance to meet my Prof. M.A. Kalam sensei, Dr. S.L. Sen sensei, Prof. Khondoker Sensei. They all asked about my training and they told me that they are eagerly waiting to receive Prof. Nozaki sensei and Prof. Sakurai sensei for the plastic surgery conference in February. Dr. Sen sensei is really excited about his visit to Japan as he eagerly wants to meet Hirayama sensei. It was very nice to hear while my staying in Bangladesh as Prof. Kalam sensei became grandfather for the first time. Two overseas surgical group from Singapore (Sing Health) and Britain (BAPRAS) was that time in Bangladesh. Had a chance to join morning conference there. It was really hard to leave my family, but I had to.

Total operations of the later part of the month was a few and mostly in outpatient department. Before going to Bangladesh had a chance to see VRAM free flap for reconstruction of tongue and floor of oral cavity .VRAM free flap was first time for me. Also I had a chance to see some maxillofacial fracture cases. Blepharoplasty and lateral tarsal fixation cases in outpatient clinic. Also some local anesthetic cases like haemangioma excision, scar revision, volume reduction after DIEP free flap, granuloma upper lip excision and other cases. Last working day was really exciting as we had a party along with operation theater staff nurses. We had dinner all together in a restaurant near our hospital and after that we went to a karaoke restaurant. The total party was great and I really want to thank Prof. Sakurai sensei, Dr. Yamaki sensei, Dr.Itoh sensei, Dr. Matsumine sensei, Dr. Fujiwara sensei for arranging such a nice party. And it is a very good tradition to have a party with other staffs. The New Year was ahead of me and also the cold!

I can feel the cold now. And of course had an experience of snowfall for the first time in my life! The difference of winter of Japan with that of Bangladesh is that usually there is no rain in winter in Bangladesh. The best part of November is that changing colors of leaves. And it’s really amazing to watch that. I had an experience of having to see the colorful leaves at Tokyo and Hiroshima. Had a chance to join Microsurgery conference in Hiroshima. I am really grateful to Prof. Nozaki sensei, Prof. Sakurai sensei and JBMA for giving me the opportunity to join the conference. Went to Miyajima Island from Hiroshima too .And had a chance to see world cultural heritage sites there. Plastic surgeon from China Dr. Li also went back to her country this month.

Hospital and Academic Activities:

The entry to operating room is changed and also the changing rooms. Joined a lot of cases. Some of those were first time for here. Continued my learning activities here at hospital. Had a chance to assist and see 6 free flap cases this month.3 DIEP cases, 2 ALT free flap cases and 1 LD free flap case. DIEP cases were for breast reconstruction, ALT cases were for soft tissue coverage of dorsum of foot and LD free flap case was for soft tissue coverage of exposed skull bone. Varicose vein surgery,# nasal bone cases, open reduction and internal fixation of # maxilla ,scar revision ,use of tissue expander, lymphovascular anastomosis cases, removal of retrosternal bar case, blepharoplasty cases, use of LD flap for breast reconstruction cases were as usual. Learning a lot from the OR. Also had a chance to see rectus abdominis myocutaneous flap for chest wall reconstruction and composite nipple and areola graft. Had a chance to join microsurgery conference at Hiroshima from November 17 to 19.A lot of lectures and presentation were there. Lectures of senior professor from Serbia, UK and USA was so much informative. Had a chance to see video presentation session. Harvesting of ALT free flap, temporalis fascia free flap, free style perforator flap, penile reconstruction with groin flap and other cases. A detailed presentation on Dupuytren contracture was in conference too. Presentation on lymphatic malformation case diagnosis and different treatment modalities was there too. At the last lesson had a chance to see presentation on free flap monitoring techniques used by different centers in Japan too. The total conference was informative and I learned a lot of new things there and also had a chance to meet new people. Dr. Li from China also left this month. We had a farewell dinner for her at a restaurant near akebonobashi station. The foods were delicious. I enjoyed the dinner. Specially Oden and Nimono.

Had a chance to visit Shinjuku and Ueno park area to see the autumn leaves. Enjoyed a lot at Shinjuku Gyoen National garden area. The different areas of park with different types of trees and flowers were amazing. The chrysanthemums beds were really appreciable. It’s a very rare art to present flowers in such a nice way. As had a chance to visit Hiroshima, so went few places there. Went to The A bomb dome, the peace memorial park, children peace monument and peace memorial museum. It was really shocking to see the devastating action of nuclear weapon. So many deaths! And so long term effects! But the credit should goes to people of Hiroshima as they have rebuild the city in such a nice way. Had a chance to taste traditional food Okonomiyaki with Prof. Sakurai sensei, Dr. Yamamoto sensei and also with Dr. Masa Hasegawa sensei. On the day of my returning I had a chance to go to beautiful Miyajima Island. The autumn leaves were so beautiful there! Also had a chance to see world cultural heritage site giant O tori gate and Itsukushima shrine there. There were wild deer in Miyajima Island. It was really pleasant trip to Hiroshima. Getting board in shinkansen for the first time was really amazing and when a view of Mt.Fuji is added from shinkansen the joy of journey of course becomes doubled.

It’s getting cooler! When rain and wind is added! It’s autumn coming. So the changing of color of leaves is coming too. The weather is actually pleasant. But I did not know what’s coming ahead. Began with meeting a great guy Prof. Perenlei Enkhbaatar from Galveston, Texas. He is the head of translation intensive care unit. He is from the same institute where Prof. Nozaki sensei, Prof. Sakurai sensei, Dr.Fujiwara sensei used to work during their USA days. And of course the ikoku is transferred to its new location at the end of this month. The great thing is that Professor’s office is under the same roof.

Hospital Activities:

Now I am a bit acquainted with the hospital environment. Continued my learning activities here at hospital. Had a chance to see new cases, refreshment cases and some new innovative cases, assisted in some operations. It was great! A lot of Breast reconstruction cases, skin grafting cases, nasal bone fracture cases, vascular cases, facial bone fracture cases, reduction mammaplasty case, cheiloplasty case, fracture maxilla case, blepharoplasty cases. Had a chance to see few cases in outpatient department too. So a variety of cases. Had a chance to go to neurosurgery operation theater to see some cases. Especially vertical trapezius case and craniosynostosis case. They also prepared 3D model of skull to have a preoperative planning. They incised and freed the area of scalp as per instruction of plastic surgery department and external distraction device was placed by plastic surgery department. This case was new to me.Could learn a lot. Prof. Perenlel Enkhbaatar delivered a lecture and it was so informative. I really enjoyed his lecture on smoke inhalation and very much happy to see the works they really do for the improvement and having new era’s in the field of burn patients’ treatment. It was really great to have such type of lecture. Another reason of my enjoying that lecture as because the total lecture was in English. We went to a restaurant after that .Had dinner and also experienced the nabe cuisine there too.

Extra academic activities:

Had a chance to visit Tokyo downtown in the weekends. Went to Ueno zoo. And for the first time I could see a panda there. Also had a chance to see Tokyo Skytree, Tokyo tower, Ginza area and Shinjuku Gyoen national garden. So it was another month of my learning process that continued .Trying to gather new things to enrich my knowledge.

The month of September was the ending of summer. The weather was variable. Sometimes it was hot, sometimes it rained all day long and sometimes it was pleasant. Trying to adjust still and had some experiences. The special day for muslim the EID UL AZHA was in this month and I missed my family. Sometimes it became so tough for me here! Also having some trouble with my sleep. The hospital and the extra-academic activities has enriched my knowledge and experiences a lot.

Hospital Activities:

Continued my learning activities here at hospital. Had a chance to see new cases, refreshment cases and some new innovative cases, assisted in some operations. It was great! There were free flap cases, Breast reconstruction cases, replantation case, parotid tumor cases, skin grafting cases, nasal bone fracture cases, vascular cases, facial bone fracture cases, infraorbital floor augmentation case, lymphovascular anastomosis cases and some other cases. Had a chance to see few cases in outpatient department too. So a variety of cases. Free DIEP flap cases were interesting too and it’s very enjoyable to make the planning out of the CT angiogram images. To see the 3D model using for planning was enjoyable. Model was prepared from 3D printer so that preoperative assessment and planning was possible. Prof. Sakurai sensei prepared the template on the model so that it can be used in the operation. Enjoyed the lymphovascular anastomosis case too. Also got acquainted with the special instrument uses during hand surgery and it is very useful too.

Also joined a burn meeting at Takadano-baba. Professor Kawakami from Kanazawa presented about his experiences and about the activities of his department in treating burn patients. He also talked about the basic management protocol of burn patients and the methods of burn wound coverage. The meeting was followed by a very pleasant buffet dinner.
Plastic surgeon from China Dr. Li came to department this month. So had a company to share with her.

Extra academic activities:

We had a party at Shinjuku where plastic surgeons from three universities joined on 8th September. I along with Dr. Li (from china) also attended. It was great meeting other doctors there.
12 September was the EID UL AZHA here in Japan.Missed my family a lot on the day. The day started for me with typical EID prayer. So had to wear typical dress Punjabi and went to Mosjid Assalam in Ueno Okachimachi.Had my prayer there and met some muslim people. Prepared some food for the day..
Had a great baseball watching with the department at Tokyo Dome stadium. The match was great to watch. Loved the enthusiastic supporters and also the match. Thanks Osada Sensei for the arrangement.

I had a chance to visit different places in Tokyo. Though it rained almost every weekdays! Visited the largest wholesale fish market of the world, the Tsukiji fish market. It was huge and lots of activities there. Had a look around both inner and outer market.

Also visited the east garden of Tokyo imperial palace and Tokyo station.

Went to Bangladesh embassy in Tokyo in the last week .Made my citizen registration there. Met my brother’s friend there.
So it was my learning process that continued through this month. Trying to gather new things to enrich my knowledge.

August- month of summer. It’s hot, humid (but not as much as Bangladesh), there were typhoons and so on. It was the first full month for me in Japan. Like that of different colors of summer I had some different experiences both in my hospital works and also outsides. Had a chance to explore new areas, met some wonderful people.

Hospital Activities:

Had a chance to see new cases, assisted in some operations. It was great! Total number was 29. Of them Free flap was 05,Breast reconstruction 04,Vascular cases 05,Chest wall 03,skin graft 04,aesthetic case 01,burn contracture 01,pressure sore 01,head neck 02,others 03 cases. So a variety of cases. Free groin flap and free expanded DP flap was new for me and DIEP flap planning, internal mammary recipient vessel preparation was also new for me. Had a great chance to learn from Prof.Sakurai sensei about DIEP flap planning. Pectus excavatum treatment with placement of retrosternal bar cases were new for me. Thanks Kikuchi Sensei for helping me to understand a bit. Varicose vein surgery with minimal access cases were 04. Yamaki sensei was very kind to me for better understanding these cases. One of the cases that I want to see more and more is lymphatic obstruction and lymphovascular anastomosis. Use of photodynamic eye was new to me. But I am very much keen to learn more about lymphatic malformation and obstruction cases. The use of deeper part of epidermis for treatment of congenital leukoplakia was interesting .Had a chance to see Use of versajet. Had a chance to see fat grafting for depressed supraorbital region, VY rotation advancement flap with deepithelized skin for ischial pressure sore, Post burn contracture in right ankle region release and placement of artificial dermis case. So variety of cases and learnt a lot too. One of overseas doctor from Italy Dr. Tomasso was here in first 3 weeks. Had a chance to talk with him and share our views too.

Extra academic activities:

August started for me with a welcome dinner from my department. It was a Turkish restaurant in Shinjuku-ku. Enjoyed the dinner. Thanks all for the dinner. Had a chance to see the traditional Japanese archery game with Prof. Sakurai sensei and Maiko San. It was very interesting. Participants used very long bow and arrow with special costumes. But the duration was very short! August 06 was a memorable day for me. As because I had a chance to meet Prof. Hirayama Sensei and Mrs. Hirayama Sensei in Meguro. Thanks my mentor Prof.Sakurai sensei for giving me that opportunity. The following hours of that day and evening was excellent. Went to see Itabashi hanabi with Fujiwara Sensei. Met Prof. emeritus Nozaki sensei, Prof. Nakazawa Sensei there. They were very cordial and kind to me. The fireworks show was great and enjoyed a lot. Had dinner in the cruise and lots of fun. Thanks Dr.Miyamoto sensei and Mrs.Miyamoto sensei for their kind invitation and hospitality. Had a chance to stroll in some areas in Tokyo. Especially Ueno park, Ameyoko market, Sumida river side in Kuramae and Tocho-mae.

So, lots of experiences for me in the month of August here in Japan. Hope to continue the Journey.

Japan-land of rising sun, land of unity, punctuality, land of strength and land of humbleness. I cannot recall when I have heard the name of this country. May be from my school books, from my parents, from my teachers or from my parents or from my collected stumps in my childhood. But I never thought that I would ever come to Japan. It was some kind of surprise for me all of a sudden. One fine morning of April my Professor Dr.Md.Abul Kalam called me in his office and told me that he has selected me for the next JBMA candidate. So I had to prepare myself within very short time with all my official papers ready. I booked my air ticket in advance though I was yet to apply for my VISA. Within that period a very sad incidence occurred in Bangladesh. Along with some foreigners seven Japanese people were killed by some miscreant terrorists. It was a horrible incidence and everyone was so panic with the incidence. I was worried about having my VISA. Sakurai sir called me to know about my coming and about my VISA. He was worried whether I will be able to get my VISA within that very short time or not. But everything was fine. I started from Bangladesh on 16 July while kept my family (my wife, my sweet daughter, my parents) shedding in tears.

The journey was fine and had a very welcome reception from my Prof. Sakurai sir and Assistant Prof. Itoh sir. Had welcome dinner with them and they helped me to my hotel. Itoh sensei took me to hotel and assured me all the advice and help. I contacted my family using the internet.

The next day was holiday in Japan. So I did not need to go to hospital. I had a 48hours subway ticket gift from Prof. Sakurai sir. So I tried to use that. Had a ride to Wakamatsu-kawada, so that I can come the next day.

The following days were so sweet. Met the department. Went to offices, operation theaters. Shionoiri San helped me a lot. Thursday and Friday was 8th annual conference on surgical wound care. Tanabe sensei helped me to go there at Ikebukuro. I met Prof. Nozaki Sensei there. He assured me about every kind of help I need. It was my pleasure to meet him. On Friday went to Nerima ward office for my residency registration. It was Maiko Kunimi San who took all the pain there. Still now she is taking care of all my troubles. In the meanwhile had a chance to go out for a dinner with Sakurai sensei. Enjoyed Japanese fish (Raw fish, fried fish, tempura etc.). So far the days were great. Hope to continue the journey.
